Hospital Discharge Report and Recommendations

Hospital discharge was raised as a potential issue with Healthwatch Greenwich, so we investigated further.

The main acute hospital in Greenwich is Queen Elizabeth Hospital which includes an Emergency Department and an Urgent Care Centre. Residents of Greenwich also use services in neighbouring boroughs, including

Poor quality discharge processes, delayed discharge, being discharged too early or not having the correct support in place prior to discharge from hospital can all have a profoundly negative effect, especially with vulnerable patients. Patients who are discharged too early are more likely to be readmitted, without suitable care in place2. Those who are discharged too late are at a higher likelihood of acquiring infections and have poorer recovery rates. Lewisham Hospital. Lewisham & Greenwich NHS Trust provide Queen Elizabeth Hospital & Lewisham Hospital services, apart from the Urgent Care Centre services which are provided by Greenbrook Healthcare.

We held a focus group, ran a survey and carried out several Enter and View visits to the QEH discharge lounge. We then produiced a series of recommendations for Lewisham and Greenwich NHS Trust (LGT). LGT responded positively to our recommendations.
